I give consent for my child to participate in Taekwon-Do training and comply with any reasonable direction of the instructors and coaches at Warrior Taekwon-Do in relation to participating in their sessions. I understand that that all Taekwon-Do related training and registration fees will be invoiced to me by Diocesan School for Girls and agree to pay these fees. I agree that my child is in a good state of health and is medically fit to participate in the exercises required for participation in this programme. I agree not to hold Warrior Taekwon-Do or any of its staff / members / coaches / volunteers responsible for any injury my child may sustain during their participation in this programme. I agree my child's likeness (for example: audio, video, photograph and others) may be published in any of Warrior Taekwon-Do's promotional material in any media (for example: website, print, cinema, video and others).
